Schedule shift coverage

FOR PROPSECTIVE CLINICS

Be advised that an independent contractor agreement is reviewed with any new client practices prior to any scheduled work. For new client practices, a single initial shift is scheduled prior to further shifts being able to be scheduled. This is to ensure that myself and your team are a good fit for each other moving forward.

A signed copy of the independent contractor agreement must be received within 3 days of scheduling a first shift. If a signed copy of the independent contractor agreement is not received within 3 days of scheduling a first shift, the shift will be cancelled and must be re-scheduled after finalizing the agreement.

HOW TO USE THIS SCHEDULER

Shorter shifts of 4-6 hours can be booked for clinics within 15 miles of Avondale, Chicago. The minimum booking is otherwise 8 hours unless otherwise arranged.

There is an additional $20 charge per shift for shift locations that are 30 miles or further from Avondale, Chicago.

Weekday hourly rate = $150/hour

Weekends and holidays or any time exceeding 10 hours per shift = the above mentioned weekday hourly rate + 50% additional cost per hour.

The hourly rate is subject to negotiation depending on the extent of expertise required. Special procedures and advanced imaging services entails a higher hourly rate.

A 1 hour lunch is included in the hourly rate for 8+ hour days.

My calendar generally operates on a 90-day rolling schedule. However, I am open to discussing specific shift requests further in advance if necessary.
