For Prospective Clinics

Thank you for your interest in scheduling relief or mobile ultrasound services.

For new clinic relationships, scheduling details are confirmed in writing and a signed agreement is required to finalize the appointment. If the signed agreement is not returned within 3 days, the scheduled date may be released.

How to Use This Scheduler

Please choose the start time only. Total shift duration can be clarified on the next page. For mobile ultrasound services, the process is similar, except you will answer N/A for end time.

Shorter shifts of 4-6 hours can be booked for clinics within 15 miles of Avondale, Chicago. The minimum booking is otherwise 8 hours unless otherwise arranged.

There is an additional $20 charge per shift for shift locations that are 30 miles or further from Avondale, Chicago.

Weekday hourly rate: $155 per hour

Weekend shifts or any time exceeding 10 hours per shift are billed at 50% above the weekday hourly rate.

Shifts occurring on holidays are charged at a rate of double the regular weekday shift rate.

Special procedures and advanced imaging services may involve additional fees. Please refer to the Rates and Policies page for current service details.

A 1 hour lunch is included in the hourly rate for 8+ hour days.

My calendar generally operates on a 90-day rolling schedule. If you are requesting dates farther in advance, please feel free to reach out directly.
